The five-time Olympic medalist became the first woman to perfect the triple-twist, double-flip move in her first pass on floor.
The Olympic champion became the first gymnast to land the double-twisting, double somersault move.
"You literally had one job and you couldn’t protect us," the Olympic medalist said at the U.S. Gymnastics Championships this week.
"I had a blackout. I don’t know why I took the wrong way," Teresa Stadlober later said of her devastating mistake.
The Dutch athletes apologized for the "very regrettable" stunt.
"It's just kind of a given that you don’t fall," NBC's Terry Gannon says.
"It's the ultimate redemption," he said.
